Step-by-Step Structure
Week 1: Initial Assessment
- Comprehensive Intake and Care Needs Evaluation:
We begin by meeting with the client and their family to thoroughly understand medical history, daily routines, preferences, and any current challenges. This assessment helps us identify immediate and long-term needs. - In-Home Safety and Wellness Check:
Our Care Navigator performs a detailed walkthrough of the home to spot potential safety risks and recommends practical adjustments to enhance comfort and prevent falls or accidents. - Personalized Care Plan Development:
Based on the intake and safety check, we develop a customized care plan outlining daily assistance, health goals, and family expectations. - Designation of a Dedicated Care Navigator:
Each client is assigned a dedicated Care Navigator who coordinates all aspects of care, ensuring consistency, personalized attention, and a single point of contact for families and other care professionals.
Ongoing Services Based on Tier Level (Monday–Friday, 8 AM – 4 PM)
- Coordination of Community Resources:
Assist in connecting clients to local resources like meal delivery programs, transportation services, senior centers, or social activities to enhance quality of life. - Oversight of Home Care and Caregiver Performance:
Our Care Navigator monitors the quality of care provided, conducts check-ins with caregivers, and ensures that services align with the care plan and adapt as needs evolve. - Wellness Education:
Provide the client and family with practical education about health conditions, medication side effects, fall prevention, or dementia care tips. - Emergency Planning and Crisis Response:
Keep an up-to-date emergency plan in place and be available during business hours to coordinate urgent care needs or arrange immediate support if something changes unexpectedly.

Tier 1: Navigation Essentials – $400/month

What Full Care Team Communication Means
Full care team communication is included with Tier 2 and Tier 3. This means your Care Navigator will coordinate and collaborate with all members involved in a client’s care to ensure everyone is informed and aligned. This includes:
